What is Cancer Antigen 19-9?
CA 19-9 is a sialylated Lewis A blood-group antigen expressed on the surface of epithelial cells and shed into the circulation. It is synthesised via fucosyltransferases whose activity depends on the Lewis blood-group genotype; individuals who are Lewis-antigen-negative (approximately 5–10 per cent of the population) cannot synthesise CA 19-9 and will have undetectable values regardless of disease state. Physiologically, low concentrations are shed by biliary, pancreatic, gastric, and colonic epithelia. Serum concentrations rise when tumour cells overexpress or shed the antigen or when biliary obstruction impairs clearance.
