What is Neurofilament Light Chain?
Neurofilament light chain is a structural protein of the neuronal cytoskeleton, expressed exclusively in axons. Following axonal injury or degeneration — whether from trauma, inflammation, ischaemia, or neurodegenerative processes — NfL is released into the cerebrospinal fluid and diffuses into blood. Its plasma concentration tracks axonal damage burden across diverse neurological conditions, and because elimination is primarily renal, levels rise modestly with age and impaired kidney function. The protein's half-life in blood is on the order of a few weeks, so plasma NfL integrates recent axonal loss rather than reflecting moment-to-moment fluctuation.
