What is Salivary Free Cortisol (Single Timepoint)?
Salivary cortisol reflects the unbound, biologically active fraction of circulating cortisol and closely tracks rapid fluctuations in hypothalamic-pituitary-adrenal axis output. Because cortisol secretion follows a pronounced diurnal rhythm — highest on waking, falling sharply through the morning, and reaching a nadir at night — each salivary sample captures the axis at that specific moment. An add-on timepoint expands the diurnal curve, improving resolution of blunted or delayed rhythms that a standard four-point collection might miss.
