What is DHEA, Salivary (Morning)?
Salivary DHEA is the free, unbound fraction of circulating unconjugated DHEA that passively diffuses across the salivary gland acinar cells into saliva — reflecting the biologically active DHEA fraction available at the tissue level, analogous to salivary cortisol's relationship to serum total cortisol. Salivary DHEA follows a diurnal pattern similar to unconjugated serum DHEA — peaking in the morning — but with less pronounced diurnal variation than cortisol and substantially lower absolute concentrations. It does not reflect the stable DHEA-S pool, which requires serum measurement. DHEA serves as an anabolic counter-regulatory hormone to cortisol's catabolic and immunosuppressive effects, making the cortisol:DHEA ratio a clinically meaningful index of the balance between stress-driven catabolism and anabolic resilience.
