What is Total Serum Calcium?
Calcium is the most abundant mineral in the body and the principal cation of bone. Approximately 99 per cent resides in the skeleton, while the remaining one per cent circulates in plasma and extracellular fluid. In serum, roughly 40 per cent is bound to albumin, 10 per cent to anions such as phosphate and citrate, and 50 per cent exists as free ionised calcium — the physiologically active fraction that governs neuromuscular excitability, enzyme cofactor activity, and signal transduction. Parathyroid hormone, vitamin D, and calcitonin tightly regulate ionised calcium through renal, intestinal, and skeletal fluxes.
