What is Total Thyroxine (Total T4)?
Total T4 is the combined measurement of protein-bound and free fractions of thyroxine in serum. Approximately 99.97% of circulating T4 is bound to thyroid binding globulin (TBG, approximately 70%), transthyretin (approximately 15%), and albumin (approximately 15%) — only the free fraction is biologically active. Total T4 is therefore substantially influenced by TBG concentration: elevated TBG from oestrogen exposure (OCP, HRT, pregnancy) raises Total T4; reduced TBG from androgen excess, corticosteroids, nephrotic syndrome, or severe liver disease lowers Total T4 — in all these states without truly reflecting thyroid hormone bioavailability. This TBG sensitivity is the primary limitation of Total T4 relative to Free T4 in the modern thyroid diagnostic landscape.
