What is 24-Hour Urine Chloride?
Chloride is the principal extracellular anion and tracks closely with sodium in renal handling. Under steady-state conditions, 24-hour urinary chloride excretion reflects dietary intake and provides an integrated index of volume status and effective circulating volume. The kidney tightly regulates chloride reabsorption along the nephron, particularly in the thick ascending limb via the Na-K-2Cl cotransporter and in the collecting duct, where chloride transport is coupled to proton secretion. When effective volume is reduced — whether through true hypovolaemia or states such as heart failure or cirrhosis — distal chloride reabsorption increases and urinary chloride falls.
