Description
What is the DUTCH Male Hormone Test?
The DUTCH Male Hormone Test from BiomarkersLabs.com is a comprehensive dried urine hormone metabolite profile specifically designed for men — measuring not only parent testosterone and its precursors, but also the downstream metabolites of testosterone and adrenal hormones that reveal how these hormones are being metabolised, converted, and cleared in the male body. Produced by Precision Analytical, the DUTCH Male test provides clinically actionable data that serum testosterone measurements alone cannot offer, making it the most comprehensive single-specimen male hormone assessment available in functional and integrative medicine.
The key clinical advantage of the DUTCH Male test over conventional serum testosterone panels is its measurement of testosterone metabolites — including the 5-alpha and 5-beta reduced metabolites (androsterone, aetiocholanolone, 5α-androstanediol glucuronide) — which reveal the balance between 5-alpha reductase activity (the enzyme that converts testosterone to the more potent dihydrotestosterone, or DHT) and 5-beta reductase activity. This is directly relevant to understanding hair loss risk, prostate androgen stimulation, and the clinical presentation of men with normal total testosterone but high DHT-driven symptoms. The test also measures the four-point diurnal cortisol curve alongside total cortisol production from metabolites, DHEA metabolites, and oestrogen metabolites — providing the complete adrenal and sex hormone metabolite picture in a single home-collectable dried urine kit. Specimens are processed at Precision Analytical’s CLIA-certified laboratory in the United States. Results delivered within 10–14 business days. Available in USA only. Licensed practitioners only.
What does the DUTCH Male Hormone Test measure?
Androgens and metabolites: Total testosterone (urinary metabolites) | Androsterone (5α-reductase pathway metabolite) | Aetiocholanolone (5β-reductase pathway metabolite) | 5α-androstanediol glucuronide (DHT tissue activity marker) | DHEA metabolites: aetiocholanolone | androsterone | Androstenedione
Oestrogen metabolites: Oestrone (E1) | Oestradiol (E2) | Oestriol (E3) | 2-OH oestrone | 4-OH oestrone | 16-OH oestrone | 2-methoxy-oestrone (methylation index)
Adrenal and cortisol markers: Free cortisol — four-point diurnal curve | Free cortisone — four-point diurnal curve | Cortisol metabolites: THF | allo-THF | THE | Total cortisol production | DHEA metabolites
Additional markers: Melatonin (MT6s — overnight urine)
Clinical indications
Comprehensive male hormone metabolite assessment — for practitioners requiring the complete picture of how testosterone and other hormones are being metabolised in a male patient — information that is entirely invisible to standard serum panels.
TRT monitoring — metabolite pattern assessment — the DUTCH Male provides information about testosterone metabolism pathways during TRT, including 5-alpha reductase activity (DHT production), oestrogenic conversion (aromatisation metabolites), and total androgen burden, complementing standard serum TRT monitoring.
Male pattern hair loss — 5-alpha reductase activity assessment — elevated 5α-reduced testosterone metabolites (androsterone relative to aetiocholanolone) indicate high 5-alpha reductase activity and elevated DHT tissue exposure, directly relevant to androgenic alopecia and prostate androgen stimulation risk.
Prostate health — androgen metabolite profiling — the balance between 5α-reductase and 5β-reductase activity influences DHT-driven prostate androgen stimulation; DUTCH Male metabolite profiling provides clinically relevant data for men with benign prostatic hyperplasia (BPH) or elevated PSA in the context of androgen assessment.
Diurnal cortisol and HPA axis assessment in men — the four-point cortisol curve plus cortisol metabolites provides the complete adrenal picture — rhythm, total production, and cortisol:cortisone ratio — for men presenting with fatigue, burnout, or stress-related symptoms alongside hormonal complaints.
Oestrogen metabolism in men on TRT — the DUTCH oestrogen metabolite panel reveals whether testosterone aromatisation during TRT is producing oestrogen metabolites through protective or potentially harmful pathways, providing metabolic safety data beyond simple serum oestradiol measurement.
Functional and longevity medicine male hormonal audit — for practitioners conducting comprehensive male hormonal health assessments in longevity, anti-ageing, or integrative medicine practice, the DUTCH Male provides the broadest and most actionable single-panel male hormonal dataset available.
Sample type and collection
Dried urine (filter paper card). No fasting required. The patient collects urine samples at four time points across a single day — waking, morning, afternoon, and bedtime — by urinating onto filter paper cards that are dried and mailed to Precision Analytical. An overnight urine collection for melatonin measurement is also included. Full instructions and all collection materials are provided in the DUTCH Male kit. Home collection is fully suitable. The kit must be ordered through BiomarkersLabs.com and shipped to the patient by the practitioner. Unlike serum testosterone, no specific timing restriction applies — the dried urine collection captures an integrated daily hormone exposure picture rather than a single-point snapshot.
Turnaround time
10–14 business days from specimen receipt at Precision Analytical laboratory.
