What is Salivary Cortisol, Diurnal Profile (multiple timed collections across the day)?
Cortisol is the principal glucocorticoid, secreted by the adrenal cortex under pituitary ACTH control in a pronounced diurnal rhythm that peaks shortly after waking and declines to a nadir in the late evening. Approximately 90 to 95% of circulating cortisol is protein-bound, chiefly to corticosteroid binding globulin, and only the free fraction diffuses passively into saliva. Salivary cortisol therefore reflects the free, biologically active fraction and is largely independent of binding protein concentration, which is the principal analytical advantage of salivary over total serum measurement. Salivary levels track serum free cortisol closely with a lag of a few minutes.
