What is Thyroid Peroxidase Antibodies?
Thyroid peroxidase (TPO) is the key enzyme responsible for the iodination of thyroglobulin tyrosine residues (organification) and the coupling of iodotyrosines to form T4 and T3 within the thyroid follicular lumen. Anti-TPO antibodies are IgG autoantibodies generated against conformational epitopes on the TPO enzyme, produced by thyroid-infiltrating B-cell plasma cells in the context of autoimmune lymphocytic thyroiditis. They are pathologically active — binding complement and mediating antibody-dependent cellular cytotoxicity (ADCC) against thyroid follicular cells — contributing directly to progressive thyroid gland destruction and the clinical hypothyroidism of established Hashimoto's thyroiditis. Anti-TPO titres tend to decline over the natural history of Hashimoto's disease as the antigen-bearing (functional) thyroid tissue is progressively destroyed.
