What is Total Triiodothyronine?
Total T3 is the combined measurement of protein-bound and free fractions of triiodothyronine in serum. Approximately 99.7% of circulating T3 is bound to TBG, transthyretin, and albumin — only the free fraction is biologically active. Total T3 is therefore significantly influenced by TBG concentration — which is elevated by oestrogens (OCP, HRT, pregnancy) and reduced by androgens, glucocorticoids, and nephrotic syndrome — limiting its utility in populations where TBG levels are altered. Total T3 has historically been used in hyperthyroidism assessment where T3 elevation is diagnostically significant — T3 toxicosis presents with elevated Total T3 and Free T3 alongside suppressed TSH and normal Free T4.
