LBP is an acute-phase protein synthesised by hepatocytes and enterocytes that binds bacterial lipopolysaccharide (LPS, endotoxin) and transfers it to CD14 and TLR4 receptors, initiating innate immune responses. It is part of the host defence against Gram-negative bacteria, but persistent elevation reflects chronic low-grade endotoxaemia and is associated with metabolic inflammation and barrier compromise.
Elevated LBP is a marker of bacterial translocation and intestinal barrier dysfunction, seen in conditions characterised by dysbiosis, increased permeability, and metabolic endotoxaemia — obesity, type 2 diabetes, non-alcoholic steatohepatitis, and inflammatory bowel disease. It correlates with systemic inflammation and cardiometabolic risk and is used to assess the functional integrity of the gut barrier.
