What is Sodium, Serum?
Sodium is the dominant extracellular cation and the primary determinant of extracellular fluid (ECF) osmolality and volume. Serum sodium is regulated by the integration of arginine vasopressin (AVP/ADH) — controlling renal free water reabsorption in the collecting duct via V2 receptors and aquaporin-2 channels — and aldosterone — driving distal tubular sodium reabsorption — in response to osmoreceptors in the hypothalamus and volume sensors in the great vessels. The kidneys can adjust urine osmolality from 50–1200 mOsm/kg to precisely maintain serum sodium within the normal range across extreme variations in water and sodium intake.
