What is Alanine Aminotransferase (ALT, SGPT)?
ALT is a cytoplasmic enzyme present predominantly in hepatocytes — the most liver-specific aminotransferase — catalysing the reversible transfer of an amino group from alanine to alpha-ketoglutarate, producing pyruvate and glutamate. It is released into the circulation when hepatocyte cell membrane integrity is compromised by injury, inflammation, or necrosis. ALT is approximately 3–5 times more specific for hepatocellular injury than AST, which is also abundant in cardiac muscle, skeletal muscle, and kidney.
