What is Cortisol (morning, ~30 minutes post-waking)?
Cortisol, the principal glucocorticoid synthesised in the zona fasciculata of the adrenal cortex, follows a circadian rhythm driven by hypothalamic-pituitary-adrenal axis pulsatility. Secretion peaks in the early morning (30–45 minutes after waking, the cortisol awakening response), maintaining glucose homeostasis, cardiovascular tone, and immune modulation during the fasting state. Morning cortisol represents the zenith of physiological output and is the most sensitive single timepoint for detecting primary adrenal insufficiency.
