What is Serum Cortisol (Waking)?
Cortisol is the principal glucocorticoid secreted by the zona fasciculata of the adrenal cortex under control of adrenocorticotropic hormone. It follows a pronounced diurnal rhythm, with peak secretion shortly after waking driven by the hypothalamic-pituitary-adrenal axis's anticipatory rise and the cortisol awakening response. This specimen captures the morning nadir immediately upon waking, before the awakening surge begins.
