What is Total Bilirubin, Serum?
Total bilirubin is the sum of unconjugated (indirect) and conjugated (direct) bilirubin in serum — the end products of haem catabolism from senescent erythrocytes processed by reticuloendothelial macrophages. Unconjugated bilirubin is transported albumin-bound to hepatocytes for glucuronide conjugation by UGT1A1; conjugated bilirubin is secreted into bile via MRP2 transporters. The CMP reports total bilirubin only — direct and indirect fractionation requires a separate order (see BML-GAS-043) when the total is elevated.
