What is Total Bilirubin?
Bilirubin is the end product of haem catabolism. Aged erythrocytes are broken down by reticuloendothelial macrophages, releasing haem, which is converted to unconjugated bilirubin. This lipophilic, albumin-bound form is transported to the liver, where hepatocytes conjugate it with glucuronic acid via UDP-glucuronosyltransferase to form water-soluble conjugated bilirubin. Conjugated bilirubin is then excreted into bile and reaches the intestinal lumen, where bacterial enzymes convert it to urobilinogen. Total bilirubin represents the sum of unconjugated and conjugated fractions in the circulation.
