What is Free Cortisol?
Free cortisol represents the unbound, biologically active glucocorticoid fraction filtered by the glomerulus and excreted unchanged in urine. Its 24-hour excretion integrates total daily cortisol secretion and reflects the diurnal rhythm more comprehensively than a single serum sample. Urinary free cortisol rises in proportion to serum free cortisol when cortisol-binding globulin is saturated, making it a sensitive marker of hypercortisolaemia.
